There are no enrollment fees with an annual membership contract. However, there is a one-time enrollment fee of $39 if you do not want to be tied to a 12 month commitment.
No. You are not under any obligation to continue your membership for any length of time unless you choose not to pay the enrollment fee.
Once you are a member, you may sign up additional family members at a discounted rate.
As long as your account remains active, your unused benefits will roll over for 12 Months.
Please visit one of our locations and request a cancellation form. Once submitted, you will be charged your next monthly payment as scheduled before your membership is terminated.
